Individualized Program Planning (IPP)

Transition planning is presented as an important component of the IPP process for students with special needs. Principles of effective transition planning and sample strategies are presented for transitions to elementary grades, Junior High, Senior High, and Post-secondary education. Checklists and planning tools are provided to assist in gathering and organizing information and actions supporting transitions.
